Rewards Club Master Agreement
This Master Agreement (“Agreement”) is entered into between Longevita London Limited (“Longevita”) and the individual registering as a member of the Longevita Rewards Club (“Member”). It sets out the terms and conditions that apply to all Longevita Rewards Club programmes. Participation in any individual programme is governed by this Agreement together with the relevant Schedule:
- Schedule 1 – Creator Programme (content-based rewards)
- Schedule 2 – Ambassador Programme (referral-based rewards)
A Member may participate in one or both programmes, provided they independently satisfy the eligibility requirements set out in the relevant Schedule. Each Schedule forms part of this Agreement and is incorporated by reference. In the event of any conflict between this Agreement and a Schedule, this Agreement shall prevail unless the Schedule expressly states otherwise in respect of a specific provision.
1. Parties and Nature of Relationship
1.1 Longevita – Longevita London Limited (Companies House No. 08332394), registered at 239–241 Kennington Lane, London SE11 5QU. Longevita acts solely as a non-clinical facilitator. It does not provide, manage, or control clinical services and does not influence clinical decision-making. All treatment decisions are made independently between the client and the clinician.
1.2 Member – The individual who completes the registration process, accepts this Agreement, and is approved to participate in one or more Longevita Rewards Club programmes.
1.3 Members participate as independent contractors. This Agreement does not create an employment, agency, franchise, or partnership relationship between Longevita and the Member. Members participate for their own account and are responsible for their own tax and regulatory obligations. Nothing in this Agreement affects any right a Member may have under consumer protection legislation where that legislation applies to them.
1.4 Participation is open only to individuals aged 18 or over with full legal capacity to enter into binding agreements under the laws of England and Wales. By registering, the Member confirms they meet this requirement.
2. Definitions
In this Agreement and all Schedules, the following terms have the meanings set out below:
- Agreement – This Master Agreement together with all Schedules accepted by the Member.
- Creator Programme – The content-based rewards programme governed by Schedule 1.
- Ambassador Programme – The referral-based rewards programme governed by Schedule 2.
- Independent Clinician – A medical professional who is independent of Longevita, registered with the relevant professional body in their jurisdiction (including the Turkish Medical Association and Turkish Dental Association), and practising at an independently operated and regulated facility.

- Schedule – A programme-specific addendum to this Agreement setting out eligibility, reward structure, and operational terms for an individual Rewards Club programme.
- Eligible Content – Content submitted by a Creator through the Members Portal which satisfies the requirements of clause 5 of Schedule 1 and in respect of which a Publicly Displayed View Count is available. Content which is not Eligible Content may be submitted and is licensed to Longevita under clause 7 of Schedule 1, but attracts no Creator Reward.
- Interactions – The aggregate number of likes and comments displayed publicly on a piece of content at the Measurement Time, excluding comments posted by the Creator and comments posted by any account operated by or on behalf of Longevita.

- Publication Date – The date on which a piece of content is first published and made publicly available on the relevant platform.
- Submission Time – The date and time at which a Creator submits a link to a piece of published content through the Members Portal, as recorded by the Members Portal.
- Publicly Displayed View Count – A count of views, however described by the relevant platform, which the platform displays on a piece of content to members of the public, without the viewer being required to log in as, or obtain permission from, the account holder.
- Reward Table – The table of Creator Reward bands set out in clause 3.3 of Schedule 1, as amended from time to time in accordance with clause 3.9 of Schedule 1.
- Views – The Publicly Displayed View Count recorded for a piece of Eligible Content at the Measurement Time.
All activity under this Agreement (including all Schedules) is strictly limited to procedures carried out outside the United Kingdom by Independent Clinicians. For the avoidance of doubt:
- no referral activity, promotional content, or Reward under this Agreement relates to procedures carried out in the United Kingdom;
- nothing in this Agreement constitutes arranging, managing, or carrying on a regulated activity; and
- all clinical services referenced in this Agreement are provided exclusively outside the United Kingdom by Independent Clinicians practising in Turkey.
3. General Eligibility and Registration
3.1 Eligibility requirements specific to each programme are set out in the relevant Schedule. General eligibility requirements applicable to all programmes are set out in this clause.
3.2 To register, the Member must complete the registration form in the Members Portal and accept this Agreement and the terms of any Schedule they wish to participate in.
3.3 Longevita reserves the right to refuse or revoke membership or programme participation at its sole discretion, including where it considers that a Member’s participation would be inconsistent with consumer protection standards, applicable advertising guidelines, or Longevita’s regulatory position.
3.4 This Agreement constitutes the entire contract between Longevita and the Member in respect of the Rewards Club. Any pre-existing arrangement is terminated upon activation of this Agreement.
3.5 Where a Member participates in both the Creator Programme and Ambassador Programme:
- Creator content must not be used to directly or indirectly solicit referrals;
- referral incentives must not be referenced or promoted within Creator content; and
- the two activities must remain operationally and commercially distinct at all times.
4. Brand Use and Advertising Compliance
4.1 Members may not use Longevita’s logo, trademarks, or proprietary content without prior written permission. Approved digital assets are available through the Members Portal.
4.2 Members must comply at all times with all applicable advertising and consumer protection standards, including:
- the CAP (Committee of Advertising Practice) Code;
- the ASA (Advertising Standards Authority) guidelines on endorsements, testimonials, and cosmetic procedure advertising; and
- the CMA (Competition and Markets Authority) guidance on online endorsements and consumer protection in digital markets.
4.3 Members must not produce or share content that is discriminatory, offensive, or constitutes hate speech.
4.4 Members must comply with all applicable laws in any promotional activity, including copyright, intellectual property, data protection, anti-spam, and consumer protection legislation.
4.5 In all promotional activity, Members must accurately describe Longevita as a medical tourism facilitator. Members must not describe or imply that Longevita is a clinical provider, medical facility, or regulated healthcare service, or make any claim about Longevita’s regulatory status that is inaccurate or misleading.
4.6 Nothing in this Agreement is intended to incentivise or encourage any individual to undergo medical treatment. Members must not present any Reward, benefit, or promotional activity as a reason to proceed with a medical procedure.
4.7 Members must not provide medical advice, clinical recommendations, or opinions on suitability for treatment. All individuals must be directed to seek independent clinical advice.
5. Confidentiality and Conduct
5.1 Members must not disclose any confidential information relating to Longevita’s business, operations, commercial terms, or clients that comes to their attention in connection with any Rewards Club programme.
5.2 Members must conduct themselves in a manner consistent with Longevita’s values and must not publish or communicate any false, misleading, or disparaging statements about Longevita, its independent service providers, or its clients.
5.3 Any conduct that, in Longevita’s reasonable opinion, is likely to damage Longevita’s reputation or regulatory position may result in immediate suspension or termination of all programme participation.
6. Termination and Suspension
6.1 Longevita may terminate, amend, or suspend this Agreement or any Schedule. Where a change is materially detrimental to a Member, Longevita will give not less than thirty (30) days’ notice before it takes effect, and the Member may terminate their participation at any time during that period without penalty. Longevita may terminate, amend, or suspend with immediate effect where required to do so by law or by a regulator, where necessary to address a risk of harm, or where the Member is in material breach of this Agreement.
6.2 Upon termination of any programme participation, the Member must immediately cease all promotional activity under that programme and remove any materials that Longevita reasonably requests be taken down.
6.3 Any Rewards that have been earned prior to termination and not yet issued will be honoured, subject to the relevant conditions having been met and no material breach of this Agreement having occurred. A Reward is earned once measurement has taken place under clause 3.4 of Schedule 1 and the applicable conditions in clause 3.2 of Schedule 1 are satisfied, or, in the case of Ambassador Rewards, once the conditions in clause 3.2 of Schedule 2 are satisfied.
6.4 Longevita may take civil or criminal legal action against any Member who commits fraud, deliberately manipulates any tracking or reward system, or knowingly violates any term of this Agreement or its Schedules. Any Rewards obtained through fraudulent or deceptive means will be reclaimed in full.
6.5 Longevita reserves the right to monitor publicly available content and referral activity to identify potential breaches of advertising, consumer protection, or regulatory standards, and may require immediate amendment or removal.
7. Limitation of Liability and Indemnity
7.1 Subject to clause 7.4, Longevita is not liable for any indirect, incidental, consequential, or special damages, including loss of profits, loss of goodwill, or loss of anticipated savings, arising in connection with this Agreement or any Schedule.
7.2 Subject to clause 7.4, Longevita’s total liability to the Member under or in connection with this Agreement and all Schedules shall not exceed the greater of (a) £500 and (b) the total Rewards paid or payable to the Member in the twelve months preceding the relevant claim.
7.3 Subject to clause 7.4, the Member shall indemnify and hold harmless Longevita and its officers, employees, and affiliates against any claims, losses, damages, penalties, or regulatory findings which are reasonably foreseeable and directly caused by the Member’s breach of this Agreement or any Schedule, including any non-compliant promotional or referral activity. The Member is not liable under this clause for any loss to the extent it is caused by Longevita’s own act or omission.
7.4 Nothing in this Agreement or any Schedule excludes or limits the liability of either party for: death or personal injury caused by negligence; fraud or fraudulent misrepresentation; any liability arising under section 2(3) of the Consumer Protection Act 1987; any liability under applicable data protection legislation; or any other liability which cannot lawfully be excluded or limited, including under the Consumer Rights Act 2015 where that Act applies to the Member. Clauses 7.1 to 7.3 take effect subject to this clause.
8. Rewards Payment
8.1 Payment Methods
Longevita will pay Rewards by one of the following methods, as selected by the Member during registration:
- E-gift card delivered to the Member’s registered email address (“Gift Card Payment”); or
- Bank transfer to the Member’s nominated bank account, whether held at a UK or non-UK financial institution (“Cash Payment”).
Payment method preferences and bank account details are provided by the Member at the point of registration. To update a payment method or bank account details after registration, the Member must contact Longevita by email at brand.partnership@longevita.co.uk. Longevita will process valid update requests within a reasonable time, but accepts no liability for Rewards dispatched to a previously registered email address or bank account before an update request has been processed.
8.2 Member Responsibility for Contact and Account Details
8.2.1 Members are solely responsible for ensuring that the email address and bank account details registered with Longevita are accurate and kept up to date at all times.
8.2.2 E-gift cards are delivered automatically to the Member’s registered email address. Once dispatched by the E-gift card provider (Amazon or similar), Longevita has no ability to recall, redirect, or reissue a gift card. Longevita accepts no liability for gift cards delivered to an incorrect or outdated email address, or for any failure in the Member’s email system to receive them.
8.2.3 Cash Payments made to a Member’s registered bank account constitute full and valid discharge of Longevita’s payment obligation. Longevita accepts no liability for payments made to an incorrect or outdated bank account where the Member has failed to notify Longevita of a change in advance.
8.2.4 Members must notify Longevita of any change to their registered email address or bank account details promptly by emailing brand.partnership@longevita.co.uk. It is the Member’s responsibility to ensure the update has been confirmed by Longevita before the next payment cycle.
8.3 Payment Timing
8.3.1 Rewards are attributed to a calendar month as set out in this clause. Creator Rewards are attributed to the calendar month in which the content was published and submitted through the Members Portal, and, subject to the content having been approved on brand review, are processed and paid during the second half of the following calendar month. By way of example, content published and submitted through the Members Portal at any point in March is paid during the second half of April. Where measurement under clause 3.4 of Schedule 1 has not been completed in time for that payment run, the Reward is processed and paid in the next following payment run. Ambassador Rewards are attributed to the calendar month in which the referred client’s procedure is completed, and are processed and paid during the second half of the following calendar month.
8.3.2 The administrative processing period during the first half of each month is required for Longevita to verify and validate all Reward claims relating to the preceding month. No Rewards will be paid during the first half of any calendar month.
8.3.3 Longevita reserves the right to delay payment in individual cases where additional verification is required, and will notify the Member if this applies.
8.4 Gift Card Payment
8.4.1 E-Gift Card Payment is the default method. If a Member does not specify a preference at registration, Rewards will be issued by gift card. The Business Member and Private Individual Member classifications in clauses 8.6 and 8.7 apply to all Rewards regardless of the payment method selected. Where a Member is a Business Member, a valid invoice meeting the requirements of clause 8.6.2 must be issued to Longevita before any Reward is issued, whether by gift card or by Cash Payment. The e-gift card will be delivered to the Member’s registered email address during the payment window set out in clause 8.3.
8.4.2 Gift cards are non-transferable, non-refundable, and subject to the E-gift card provider’s own terms and conditions. Longevita accepts no liability for any failure or limitation in the gift card service.
8.5 Cash Payment – General
8.5.1 Where a Member selects Cash Payment, the documentation required depends on whether the Member is a Business Member or a Private Individual Member, as set out in clauses 8.6 and 8.7 below.
8.5.2 All Cash Payments are calculated and denominated in Pounds Sterling (GBP). Where a Member holds a non-UK bank account, Longevita may at its discretion make payment in the local currency of that account. Where it does so, the total amount payable by Longevita, taken together with all currency conversion costs, foreign exchange fees, and international transfer charges, shall not exceed the GBP amount of the Reward, and all such costs are borne by the Member and deducted from or absorbed within the Reward amount. Where payment is made in GBP, all currency conversion costs, foreign exchange fees, and any international transfer charges imposed by the receiving bank or any intermediary bank are borne solely by the Member, and Longevita accepts no liability for any reduction in value resulting from conversion or transfer fees.
8.5.3 For non-UK bank accounts, the Member must provide all details required to effect an international GBP transfer at the point of registration, including IBAN, BIC/SWIFT code, and the full name and address of the receiving bank. Longevita reserves the right to request additional bank account details or verification before processing payments to non-UK accounts.
8.6 Cash Payment – Business Members
8.6.1 A Member is a Business Member for the purposes of this clause if they are operating as a sole trader, partnership, limited liability partnership, or limited company, or if they are registered for VAT or its local equivalent in respect of the activities giving rise to the Reward.
8.6.2 Business Members must issue a valid invoice to Longevita London Limited before any Reward is issued, whether by gift card or by Cash Payment. The invoice must include:
- the Member’s full legal name or company name and registered or trading address;
- the Member’s VAT or equivalent tax registration number, if applicable;
- a description of the services rendered (e.g. “content creation services” or “referral introduction services”, as applicable);
- the Reward amount in GBP;
- the invoice date and a unique invoice reference number; and
- the Member’s bank account details for payment, including IBAN and BIC/SWIFT code or similar depending on the jurisdiction where the account is held outside the United Kingdom.
8.6.3 All rewards payable under the Rewards Club are fixed amounts inclusive of any VAT, sales tax, GST, withholding tax or other applicable taxes, duties or charges. Longevita shall not be liable to pay any additional amount in respect of such taxes.
8.7 Cash Payment – Private Individual Members
8.7.1 Where a Member is not operating as a business (“Private Individual Member”), no invoice is required for any payment method. Longevita will generate an internal payment record to document the expense using the information the Member has provided at registration.
8.7.2 By providing bank account details and selecting Cash Payment, the Private Individual Member confirms that:
- they are not VAT-registered (or registered for an equivalent tax) in respect of the activities giving rise to the Reward; and
- they are not operating through a business entity in respect of those activities.
8.7.3 If a Private Individual Member’s circumstances change such that they become a Business Member, they must notify Longevita immediately by emailing brand.partnership@longevita.co.uk and comply with clause 8.6 from that date.
8.8 Tax Responsibility
8.8.1 Rewards may constitute income or a taxable benefit for the Member, depending on the Member’s individual circumstances and the jurisdiction in which they are resident, and regardless of the payment method selected or the form in which the Reward is received. Members are solely responsible for:
- declaring all Rewards to the relevant tax authority in their jurisdiction (including HMRC for UK-resident Members);
- paying any applicable income tax, corporation tax, capital gains tax, national insurance contributions, or equivalent obligations arising from Rewards received; and
- obtaining independent tax advice where needed, including in respect of any cross-border or withholding tax implications.
8.8.2 Longevita accepts no liability for any tax or social security obligations arising from Rewards paid to Members. The payment method and the currency of the Member’s bank account do not affect the Member’s tax position.
8.8.3 Longevita reserves the right to deduct any amounts required by law (including VAT, withholding tax etc.) from Rewards where it is legally obliged to do so, and will notify the Member if this applies.
8.9 Minimum Threshold and Accumulation
8.9.1 Longevita may set a minimum payment threshold below which individual Reward amounts will be accumulated until the threshold is reached. Any applicable threshold will be displayed in the Members Portal and will not exceed £25. Accumulated balances remain payable to the Member and are not forfeited by reason of the threshold.
8.9.2 Where a Member’s participation is terminated before an accumulated balance reaches the threshold, any balance will be paid out within thirty (30) days of termination provided all conditions for payment have been met.
9. Advertising Disclosure – Mandatory
Members must, in respect of every piece of content published under this Agreement:
- include ‘#ad’ prominently and at the start of the caption or description, not buried within a block of other hashtags;
- where the content is video, display ‘#ad’ or ‘Advert’ on screen from the beginning of the video and for long enough to be read by an ordinary viewer;
- in addition, use the platform’s own paid partnership or branded content label wherever the platform provides one. Use of a platform label is not a substitute for ‘#ad’;
- ensure disclosures are visible without the viewer expanding text, scrolling, or clicking through;
- ensure disclosures are in the same language as the content; and
- maintain the disclosure for as long as the content remains published, and not remove, obscure, or edit it after publication.
Members must not target content published under this Agreement at persons under the age of 18, and must not use platform targeting tools to direct such content to audiences under that age.
Failure to include required disclosures constitutes a breach of the CAP Code and CMA guidance and will result in content being rejected and no Reward being payable in respect of it. Longevita may require a Member to amend or remove non-compliant content immediately, and the Member must comply without delay.
Longevita remains responsible for its own regulatory obligations. As between Longevita and the Member, the Member is responsible for ensuring that their disclosures meet current regulatory requirements, and shall indemnify Longevita against any loss, fine, or penalty arising from the Member’s failure to do so.
10. Governing Law and Jurisdiction
This Agreement and all Schedules are governed by and construed in accordance with the laws of England and Wales. Any dispute arising under or in connection with this Agreement shall be subject to the exclusive jurisdiction of the courts of England and Wales.
11. Amendments
Longevita may amend this Agreement or any Schedule where necessary to reflect a change in law or regulatory guidance, a change to the programmes or the Members Portal, or a change in the cost or structure of the Rewards Club. Amendments will be communicated to Members via the Members Portal or by email. Amendments that are materially detrimental to Members take effect not less than thirty (30) days after notification, during which period a Member may terminate their participation without penalty; all other amendments take effect on notification. Amendments do not affect Rewards already earned, and the Reward Table applicable to a piece of content is the table in force at its Publication Date.
12. Data Protection
12.1 Longevita is the controller in respect of personal data processed under this Agreement and will process that data in accordance with its Privacy Policy (available on the Longevita Websites) and applicable data protection legislation, including the UK General Data Protection Regulation and the Data Protection Act 2018.
12.2 Longevita processes Members’ personal data, including name, contact details, bank account information, payment history, social media account identifiers, and content submitted under a Schedule, on the following lawful bases:
- performance of this Agreement, in order to administer the Rewards Club, assess and measure submitted content, and process payments;
- compliance with legal obligations, including tax, accounting, and record-keeping requirements; and
- legitimate interests, in order to monitor for fraud and misuse, to verify the integrity of reported metrics, and to operate and improve the programmes.
12.3 Content submitted under Schedule 1 may reveal information about a Member’s health, including the fact that they have undergone or intend to undergo a procedure. This is special category personal data. Longevita processes and publishes such content only on the basis of the Member’s explicit consent, which is obtained separately from acceptance of this Agreement at the point of registration, and which may be given or refused independently for:
- use of the content on Longevita’s own websites and social media channels; and
- use of the content in paid advertising, including advertising placed on third-party platforms.
12.4 A Member may withdraw their consent under clause 12.3 at any time, by the method set out in the Members Portal, and withdrawal is as straightforward as giving consent. On withdrawal, Longevita will cease further use of the relevant content and will remove it from channels under Longevita’s control within thirty (30) days. Longevita is not required to recall content already distributed to third parties, already incorporated into printed or physical materials, or placed in advertising already booked and paid for, and is not required to withdraw content where retention is necessary to comply with a legal obligation or to establish, exercise, or defend legal claims. Withdrawal of consent does not affect Rewards already earned and does not require repayment of any Reward.
12.5 Longevita may transfer personal data, including content, to Independent Clinicians, group companies, and service providers located outside the United Kingdom, including in Turkey. Where it does so, Longevita will put in place appropriate safeguards as required by applicable data protection legislation, including the International Data Transfer Agreement or the UK Addendum to the European Commission standard contractual clauses, together with a transfer risk assessment.
12.6 Members retain their rights under data protection legislation, including rights of access, rectification, erasure, restriction, objection, and portability. Requests may be made to the contact address in the Privacy Policy.
12.7 Members must handle any personal data of referred individuals in accordance with applicable data protection law and must not share such data with Longevita except through the Members Portal in accordance with the referral process in the relevant Schedule. In particular, Members must not submit an individual’s details without that individual’s knowledge and agreement, must inform the individual that their details are being passed to Longevita and that the Member may receive a Reward, and must not send unsolicited electronic marketing of any kind.
13. General
13.1 Entire Agreement. This Agreement and all Schedules accepted by the Member constitute the entire agreement between the parties in respect of the Rewards Club and supersede all prior arrangements, representations, or understandings.
13.2 Severability. If any provision of this Agreement is found to be invalid, unlawful, or unenforceable, it shall be severed to the minimum extent necessary and the remaining provisions shall continue in full force and effect.
13.3 Waiver. No failure or delay by Longevita in exercising any right under this Agreement shall constitute a waiver of that right.
13.4 Third Parties. This Agreement does not confer any rights on any third party under the Contracts (Rights of Third Parties) Act 1999.
13.5 Assignment. Members may not assign or transfer any rights or obligations under this Agreement without Longevita’s prior written consent. Longevita may assign its rights and obligations to any group company or successor entity without consent.
13.6 Notices. Any notices required under this Agreement may be sent by Longevita via the Members Portal or by email to the Member’s registered email address. Notices from the Member to Longevita must be sent to brand.partnership@longevita.co.uk.
Acceptance
By registering for or participating in the Rewards Club, the Member confirms that they have read, understood, and agree to be bound by this Master Agreement and the terms of any Schedule in which they elect to participate. Registration for the Rewards Club constitutes a binding agreement under the Electronic Communications Act 2000 and, where applicable, the eIDAS Regulation.
Creator Programme Terms (Schedule 1)
Content-Based Rewards · Forms part of the Longevita Rewards Club Master Agreement
This Schedule sets out the terms applicable to the Longevita Creator Programme. It must be read together with the Master Agreement, which governs all general obligations, conduct, liability, payment, and governing law provisions. Defined terms used but not defined in this Schedule have the meanings given in the Master Agreement.
1. Purpose and Distinction from the Ambassador Programme
1.1 The Creator Programme is a standalone content-based rewards programme, entirely separate from the Ambassador Programme. It is designed for clients who are in the process of their Longevita journey and wish to document and share their experience through original content.
1.2 The key distinction between the two programmes is as follows:
- Creator Rewards are based on content performance – Creators earn rewards for producing and publishing approved content documenting their Longevita journey, regardless of whether that content generates referrals.
- Ambassador Rewards are based on referral conversion – Ambassadors earn rewards when individuals they refer complete a journey through Longevita.
1.3 A Member may participate in both programmes simultaneously, provided they independently satisfy the eligibility requirements of each. Where a Member participates in both, this Schedule applies to all Creator content and Creator Rewards, and Schedule 2 applies to all referral activity and Ambassador Rewards.
2. Eligibility
2.1 The Creator Programme is open to individuals who have:
- attended a consultation with an Independent Clinician facilitated by Longevita and received confirmation of medical suitability for their intended procedure from that Independent Clinician;
- paid the Medical Suitability Fee, demonstrating a commitment to proceed with the procedure; and
- signed the Longevita Terms and Conditions in respect of their own treatment journey.
2.2 Completion of the procedure is not required to join the Creator Programme. Creators may begin sharing content from the point at which all conditions in clause 2.1 are satisfied. Creator status may be extended as the Creator progresses through their client journey, including after their procedure is completed, subject to continued compliance with this Schedule and the Master Agreement.
2.3 Longevita reserves the right to approve all Creator Programme applicants at its sole discretion.
3. Programme Overview and Rewards
3.1 Basis of reward. Creator Rewards are calculated by reference to the number of Views recorded for each individual piece of Eligible Content at the Measurement Time, applied against the Reward Table at clause 3.3, and are subject to the eligibility conditions at clause 3.2 and the monthly cap at clause 3.6.
3.2 Eligibility conditions. No Creator Reward is payable in respect of any piece of content unless each of the following conditions is satisfied:
- the content is Eligible Content;
- the content records 1,000 Views or more at the Measurement Time;
- the content records an Interaction Rate of 1% or more at the Measurement Time;
- the content, and the account on which it is published, have remained continuously and publicly available, un-archived, and free from material edit or re-upload, from the Publication Date until the Measurement Time; and
- Longevita has completed its brand review of the content and confirmed approval in accordance with clause 3.5.
For the avoidance of doubt, content recording 1,000 Views or more but an Interaction Rate below 1% attracts no Reward, and content recording an Interaction Rate of 1% or more but fewer than 1,000 Views attracts no Reward.
3.3 Reward Table. Subject to clause 3.2, the Reward payable in respect of each individual piece of Eligible Content is:
| Views recorded at the Measurement Time | Fixed Reward |
| 0 – 999 | No Reward payable |
| 1,000 – 2,499 | £10 |
| 2,500 – 4,999 | £20 |
| 5,000 – 9,999 | £30 |
| 10,000 – 14,999 | £50 |
| 15,000 – 19,999 | £75 |
| 20,000 – 29,999 | £100 |
| 30,000 – 49,999 | £150 |
| 50,000 – 74,999 | £200 |
| 75,000 – 99,999 | £250 |
| 100,000 – 149,999 | £300 |
| 150,000 – 199,999 | £400 |
| 200,000 – 299,999 | £500 |
| 300,000 – 399,999 | £650 |
| 400,000 – 499,999 | £750 |
| 500,000 – 599,999 | £850 |
| 600,000 – 699,999 | £950 |
| 700,000 – 799,999 | £1,050 |
| 800,000 – 899,999 | £1,150 |
| 900,000 – 999,999 | £1,250 |
| 1,000,000+ | £1,500 |
3.4 Measurement.
3.4.1 Views and Interactions are recorded at the Measurement Time, being fourteen (14) days after the Submission Time. Measurement is taken once only. No re-measurement is carried out, and no adjustment is made, in respect of any subsequent increase or decrease in Views or Interactions.
3.4.2 Longevita will use reasonable endeavours to complete its brand review of submitted content before the Measurement Time, being within fourteen (14) days of the Submission Time.
3.4.3 Where brand review has not been completed by the Measurement Time, or where measurement is otherwise delayed, measurement instead takes place at the date and time at which it is in fact carried out, by reference to the Views and Interactions publicly displayed at that time.
3.4.4 Where measurement cannot be taken at the Measurement Time by reason of platform outage, technical failure, or any other cause outside Longevita’s reasonable control, measurement will be taken as soon as reasonably practicable thereafter and the figures so recorded will apply.
3.4.5 Longevita’s determination of Views, Interactions, and Interaction Rate is final save in the case of manifest error. A Member wishing to dispute a determination must do so in writing to brand.partnership@longevita.co.uk within fourteen (14) days of the Reward being notified in the Members Portal.
3.5 Brand review. All Creator Rewards are subject to Longevita’s brand review prior to being recognised and issued. Longevita’s approval is limited to brand positioning and brand guideline compliance only. It does not constitute approval for advertising, clinical, or regulatory compliance purposes. Longevita will notify the Member of the outcome of brand review. Where content is not approved, no Reward is payable in respect of that content.
3.6 Monthly cap. The total Creator Rewards payable to any Member in respect of a calendar month, including any additional bonuses under clause 3.8, shall not exceed £3,000. Rewards are attributed to the calendar month in which the content was published. Any amount which would otherwise be payable in excess of that cap is forfeited and is not carried forward to any subsequent month.
3.7 Cross-posting and duplication.
3.7.1 Where the same content is published on more than one platform, each publication is assessed separately and each may attract a Reward, provided that each publication independently satisfies the conditions at clause 3.2. Each publication has its own Publication Date, Submission Time, and Measurement Time.
3.7.2 Where the same or substantially similar content is published more than once on the same platform, or on more than one account controlled by or operated on behalf of the Creator on the same platform, only one Reward is payable in respect of that content on that platform.
3.8 Additional bonuses. Longevita may from time to time offer additional bonuses, which may be offered on a flat-fee or per-content basis and may be amended or withdrawn at any time without prior notice. Additional bonuses count towards the monthly cap at clause 3.6.
3.9 Amendment of the Reward Table. Longevita may amend the Reward Table at any time. Any amendment applies only to content with a Publication Date falling on or after the date on which the amendment is notified to Members. Content published before that date is rewarded in accordance with the Reward Table in force at its Publication Date.
3.10 Integrity of metrics.
3.10.1 The Member warrants that all Views and Interactions recorded in respect of their content are organic and have not been generated, in whole or in part, by paid promotion or amplification, purchased views, likes or comments, engagement exchange arrangements, automated accounts, or any other artificial means.
3.10.2 Longevita may disregard, in whole or in part, any content whose recorded metrics it reasonably considers to be anomalous or inconsistent with the Member’s ordinary audience and reach, and may require the Member to provide platform analytics or other evidence before a Reward is issued.
3.10.3 Any Reward paid in respect of content which is subsequently found to breach clause 3.10.1 is repayable on demand and may be set off against any Reward otherwise payable to the Member.
3.11 Creator Rewards are paid solely for content creation services and are not linked to any clinical decision, treatment outcome, or healthcare service. No Reward is offset against, or taken into account in determining, any fee payable by the Member in respect of their own treatment.
4. Payment of Creator Rewards
4.1 Creator Rewards are paid in accordance with clause 8 of the Master Agreement. Members must specify their preferred payment method at registration. All Cash Payments are made in GBP; Members holding non-UK bank accounts bear all associated conversion and transfer costs as set out in clause 8.5.2 of the Master Agreement.
4.2 Creator Rewards attributed to a given calendar month in accordance with clause 3.6 and clause 8.3 of the Master Agreement will be processed and paid during the second half of the following calendar month. Where measurement has not been completed in time for that payment run, the Reward is processed and paid in the next following payment run. This schedule reflects the administrative processing and verification period required following measurement.
4.3 Non-compliant content will not qualify for Rewards. Longevita reserves the right to withhold or reclaim Rewards where a breach of this Schedule or the Master Agreement is identified after initial approval.
4.4 Members are solely responsible for their tax obligations in respect of Rewards received, as set out in clause 8.8 of the Master Agreement. This applies regardless of the payment method selected, the form of Reward received, and the Member’s country of residence.
Tax note: Rewards – whether received as e-gift cards or GBP bank transfer – may constitute taxable income depending on the Member’s circumstances and jurisdiction, and may need to be declared to the relevant tax authority. Members based outside the United Kingdom should take independent advice on any cross-border tax implications.
5. Content Submission and Limits
5.1 There is no limit on the number of pieces of content a Creator may submit in a calendar month. Rewards remain subject to the eligibility conditions at clause 3.2 and to the monthly cap at clause 3.6.
5.2 Content is Eligible Content where, and only where, the platform on which it is published displays a Publicly Displayed View Count for that content. This applies regardless of content format. Where a platform displays a Publicly Displayed View Count for static images, carousels, slideshows, or photo-mode posts, such content is Eligible Content on the same basis as video content. Where no Publicly Displayed View Count is available, the content is not Eligible Content, because the reach of the content cannot be measured.
5.3 The following are not Eligible Content and attract no Reward:
- content for which no Publicly Displayed View Count is available;
- stories and other content which expires or ceases to be publicly available before measurement takes place; and
- content published on a private account, or on an account which is set to private or restricted at any time before measurement takes place.
Content falling within clause 5.3 may be submitted through the Members Portal and is licensed to Longevita in accordance with clause 7, but attracts no Creator Reward.
5.4 All submissions must be made through the official Members Portal. Submissions via email, direct message, or any other channel will not be reviewed or rewarded.
5.5 Content must be published before it is submitted. The Creator must submit the link to the published content through the Members Portal within forty-eight (48) hours of its Publication Date. No Reward is payable in respect of content submitted after that period. The Submission Time recorded by the Members Portal determines the Measurement Time.
5.6 Each piece of submitted content must:
- tag the relevant official Longevita account – Instagram (@longevitauk; @longevitahairtransplant; @longevitadental); TikTok: @longevita; YouTube: @longevita;
- include the hashtag #MyLongevitaJourney and ‘#ad’ or ‘#advert’ as required under the advertising disclosure requirements in clause 9 of the Master Agreement;
- mention “Longevita” verbally or in text within the first five seconds of any video content;
- be experience-based, authentic, and reflective of the Creator’s own personal journey at the relevant stage; and
- comply in full with the content standards in clause 6.
6. Content Standards and Restrictions
6.1 General Standards
All content must be original, truthful, high-quality (minimum 720p for video), and free from misleading, exaggerated, or unsubstantiated statements.
6.2 Journey Stage Accuracy
Creators must accurately represent the stage of their journey at the time of publication. Depending on their stage, Creators may document:
- their research and decision-making process;
- their experience of the Longevita facilitation process;
- their consultation experience, without identifying the Independent Clinician or facility;
- their preparation and anticipation ahead of the procedure; and
- following completion, their recovery and post-procedure experience, subject to clause 6.6.
Creators must not predict or speculate about clinical outcomes in pre-procedure content. Creators must not promote, recommend, or encourage others to undergo any medical procedure prior to completing their own procedure and an appropriate recovery period. Pre-procedure content must be limited to personal experience only and must not contain persuasive or promotional messaging.
6.3 Mandatory Accuracy Requirements
All content must accurately reflect that:
- Longevita is a medical tourism facilitator, not a clinical provider or regulated healthcare service;
- all procedures are performed by Independent Clinicians at independently operated and regulated facilities;
- the Creator attended a consultation with an Independent Clinician who independently confirmed medical suitability before any procedure was agreed;
- any reference to outcomes must clearly reflect that results vary between individuals and are not guaranteed;
- content must not exploit insecurities, body image concerns, or vulnerabilities in order to encourage engagement with cosmetic procedures; and
- content must not be presented as medical advice and must encourage viewers to seek independent clinical advice.
6.4 Clinician and Facility Anonymity
Content must not identify, name, display, or reference any independent clinic, hospital, medical facility, or individual clinician or surgeon involved in the Creator’s treatment or consultation – including via visual content, verbal references, written text, or geotags – beyond what is published on Longevita’s website. Full details of Longevita’s Independent Clinicians are published on Longevita’s website.
6.5 Approved Content – Examples
- “I’ve done my research and I’m really excited about starting this journey.”
- “The consultation was thorough and I felt reassured before committing.”
- “The Longevita team has coordinated everything – I’ve felt supported at every step.”
- “I had a consultation and was confirmed as a good candidate.”
- “I’m now in recovery and feeling well looked after by the clinical team.”
6.6 Prohibited Content
- statements implying guaranteed results (“guaranteed results”, “permanent cure”, “risk-free surgery”, or similar);
- claims of superiority (“best clinic”, “number one surgeon”);
- any description of Longevita as a clinical provider, hospital, or regulated service;
- statements constituting medical advice or clinical recommendations;
- prediction of or speculation about clinical outcomes prior to the procedure;
- before-and-after imagery contravening CAP Code rules on cosmetic intervention advertising;
- identification of any clinician, surgeon, or medical facility; and
- competitor comparisons, discriminatory material, or content likely to bring Longevita into disrepute.
6.7 Longevita’s Review – Scope and Limitations
Longevita’s review of submitted content is limited to assessing consistency with Longevita’s brand positioning and brand guidelines only. It does not constitute a review, endorsement, or clearance of content for advertising compliance, clinical accuracy, or regulatory purposes. Creators are solely and independently responsible for ensuring their content complies with all applicable advertising standards, consumer protection law, and regulatory requirements before publication.
7. Content Rights and Usage
7.1 By submitting content, the Creator grants Longevita a non-exclusive, royalty-free, perpetual, irrevocable, worldwide, transferable licence, with the right to sublicense to Longevita’s group companies, agencies, media partners, and service providers, to use, reproduce, adapt, edit, publish, and distribute the content in any format and in any medium, including paid advertising, for the purposes of promoting Longevita’s services. This licence survives termination of this Agreement and of the Creator’s participation, and is subject to clause 12.4 of the Master Agreement.
7.2 The Creator retains copyright ownership of the content. Longevita will credit the Creator where reasonably practicable. To the extent permitted by law, the Creator waives all moral rights in the content arising under Chapter IV of the Copyright, Designs and Patents Act 1988 or any equivalent legislation, including the right to be identified as author and the right to object to derogatory treatment of the work.
7.3 Longevita may suggest edits solely for brand consistency and guideline compliance. Such suggestions do not constitute advertising or regulatory advice and do not transfer compliance responsibility to Longevita.
7.4 The Creator consents to Longevita using the Creator’s name, image, likeness, voice, and testimonial, as they appear in the content, in connection with the promotion of Longevita’s services, on the same terms as the licence in clause 7.1.
7.5 The Creator warrants that all submitted content is their own original work; that it does not infringe any third-party intellectual property rights; that it contains no defamatory or unlawful material; and that the Creator has obtained all rights, licences, consents, and permissions necessary for Longevita to exercise the licence in clause 7.1, including in respect of any music or audio, any other individual appearing or audible in the content, and any location, premises, or artwork shown.
7.6 The Creator acknowledges that audio made available through a platform’s own music library is generally licensed for organic use on that platform only. The Creator must not use such audio in content submitted for Reward without first notifying Longevita, and Longevita may require the audio to be replaced before the content is used.
7.7 The Creator shall indemnify Longevita against any claim, loss, or liability arising from a breach of the warranties in clause 7.5.
8. Creator Compliance Responsibility
8.1 Longevita’s content approval process is limited to brand positioning and brand guideline compliance. It does not constitute a review for advertising, clinical, or regulatory compliance and must not be relied upon as such.
8.2 Creators are responsible for obtaining their own independent advice on advertising compliance where needed, including in relation to the CAP Code, ASA guidelines, and CMA guidance.
8.3 Sole responsibility for advertising compliance, clinical accuracy, and regulatory adherence rests with the Creator at all times.
Ambassador Programme Terms (Schedule 2)
Referral-Based Rewards · Forms part of the Longevita Rewards Club Master Agreement
This Schedule sets out the terms applicable to the Longevita Ambassador Programme. It must be read together with the Master Agreement, which governs all general obligations, conduct, liability, payment, and governing law provisions. Defined terms used but not defined in this Schedule have the meanings given in the Master Agreement.
1. Purpose
The Ambassador Programme rewards eligible Members (“Ambassadors”) who refer new prospective clients to Longevita through legitimate promotional activity, based on their own genuine personal experience of the full Longevita client journey.
2. Eligibility
2.1 The Ambassador Programme is open only to individuals who have completed their own Longevita client journey in full. An applicant is eligible only where they have:
- attended a consultation with an Independent Clinician introduced via Longevita and received confirmation of medical suitability prior to that procedure;
- signed the Longevita Terms and Conditions in respect of their own treatment journey;
- undergone and completed an eligible cosmetic surgery, hair transplantation, or dental procedure performed by that Independent Clinician and facilitated by Longevita; and
- paid in full for that procedure, with no part of the amount paid having been refunded, reversed, or charged back.
2.2 For the avoidance of doubt, an individual who has paid the Medical Suitability Fee for, or paid in full for, a procedure which has not yet been carried out is not eligible for the Ambassador Programme. Such individuals may be eligible for the Creator Programme under Schedule 1.
2.3 Longevita reserves the right to determine eligibility and may refuse or revoke Ambassador status at its sole discretion.
3. How the Ambassador Programme Works
3.1 Referral Process
Ambassadors are provided with a Unique Web Link (UWL) – a personalised URL for exclusive use in their promotional activity. Anyone who clicks the link may complete a contact form on Longevita’s websites, generating a record in Longevita’s CRM. Longevita will provide the prospective client with a free discovery call and a price estimate.
3.2 Mandatory Conditions for Reward Eligibility
No referral reward is payable unless all of the following conditions are satisfied:
- the lead attends a consultation with an Independent Clinician facilitated by Longevita;
- the Independent Clinician assesses and confirms that the lead is medically suitable for the procedure being sought;
- the lead pays for the applicable procedure; and
- the lead has not previously contacted Longevita through any other channel or source.
For the avoidance of doubt: the referral reward becomes payable only after the Independent Clinician has exercised their professional clinical judgment and confirmed suitability. Longevita has no role in, and no authority to influence, that clinical determination.
Ambassadors must not encourage, pressure, or induce any individual to undergo a medical procedure and must clearly communicate that all decisions must be based solely on independent clinical advice.
Ambassadors must not promote or advertise the existence of referral rewards, financial incentives, or discounts as a reason to undergo treatment.
3.3 Manual Referrals
Ambassadors may submit referrals manually via the Members Portal without using their UWL. Manually submitted leads must quote the Ambassador’s details during their first contact with Longevita. Manual leads are not eligible for the £10 lead reward in clause 4.2.
3.4 Exclusions
Non-surgical procedures, revision surgeries, and dental treatments involving fewer than 16 units are not eligible. Ambassadors earn rewards only on a referred client’s first procedure facilitated by Longevita.
4. Rewards
4.1 Procedure Referral Reward
Subject to all conditions in clause 3.2 being satisfied, the Ambassador receives a £200 reward per qualifying referral. The referred client may receive a £200 discretionary promotional discount.
4.2 Lead Generation Reward
Ambassadors may also receive a £10 reward per lead generated through their UWL, provided the lead submits photographs and completes a discovery call with Longevita. Not available for manually submitted leads.
4.3 Offer Combinations
The £200 discretionary client discount cannot be combined with any other promotion, discount, or special offer. Where multiple offers may apply, Longevita reserves the right to determine which single offer applies.
5. Payment of Ambassador Rewards
5.1 Ambassador Rewards are paid in accordance with clause 8 of the Master Agreement. Members must specify their preferred payment method at registration. All Cash Payments are made in GBP; Members holding non-UK bank accounts bear all associated conversion and transfer costs as set out in clause 8.5.2 of the Master Agreement.
5.2 Ambassador Rewards for referrals in which the referred client’s procedure is completed in a given calendar month will be processed and paid during the second half of the following calendar month. For example, where a referred client’s procedure is completed in March, the Ambassador Reward will be paid during the second half of April. This schedule reflects the administrative processing and verification period required during the first two weeks of each month.
5.3 Members are solely responsible for their tax obligations in respect of Rewards received, as set out in clause 8.8 of the Master Agreement.
6. Exclusivity
6.1 During the term of their participation, Ambassadors must not promote, for payment or other incentive, any business offering medical tourism facilitation services for cosmetic surgery, hair transplantation, or dental treatment to residents of the United Kingdom or Ireland. Nothing in this clause prevents an Ambassador from describing their own personal experience of any provider.
6.2 During the term of their participation and for six (6) months following its end, Ambassadors must not use any Longevita materials, unique warm lead link, referral data, or list of contacts obtained through the Ambassador Programme to promote or solicit business for any such competitor, and must not solicit any individual whom they referred to Longevita, or who is to their knowledge a Longevita client, for the purposes of any such competitor.
6.3 Clause 6.2 does not apply where Longevita terminates the Ambassador’s participation other than for material breach by the Ambassador.
7. Referral Tracking
Longevita records referral details including the lead’s name, telephone number, and email address. The UWL identifies the referring Ambassador, and all leads are tracked and credited through the Members Portal.
8. Ambassador Content and Promotion
8.1 Ambassadors must accurately represent the Independent Clinicians listed on Longevita’s website – including their professional registrations and regulatory body reference numbers – as the parties responsible for all clinical services. Content must not name or identify individual clinicians or facilities beyond what is published on Longevita’s website.
8.2 Where Ambassadors describe the client journey in promotional content, they must accurately represent that: (i) all prospective clients are required to attend a consultation with an Independent Clinician; (ii) medical suitability is determined solely by that clinician; and (iii) no procedure proceeds without the clinician’s independent assessment and approval.
8.3 All advertising disclosure requirements set out in clause 9 of the Master Agreement apply to all Ambassador promotional activity. Ambassadors must clearly disclose their relationship with Longevita and any financial benefit receivable in every piece of promotional content.
8.4 Ambassadors bear sole responsibility for ensuring their promotional content complies with all applicable advertising standards, consumer protection law, and regulatory requirements. Longevita’s approval of an Ambassador’s participation in the programme does not constitute approval of any specific piece of promotional content.
